Please note that the Office of Vital Statistics only maintains vital records for births, deaths, and marriages that have occurred in the City of Passaic. Therefore, this office does not maintain records for births, deaths, or marriages that have occurred in other municipalities in the County of Passaic. Such requests must be directed to the municipality where the birth, death, or marriage took place.
The Office of Vital Statistics accepts applications by mail and online for copies of vital records.

Vital Record Processing Times: Five weeks
For Mail orders: fees are paid to the "City of Passaic" by money order/cahier’s check or cash only! No personal checks accepted!
Orders by mail must include a prepaid stamped envelope (addressed to the requestor).
Corrections to a vital record and marriage license applications are processed by appointment only! For appointments, please email vital@cityofpassaicnj.gov.
